School Age Case Manager/ Service & Support Administrator **MEMBERS ONLY**SIGN UP NOW***Wed, 26 Mar 2025 10:49:28 -0500 Developmental Disabilities – School Age Case Manager/Service & Support Administrator Qualifications: Successful candidates Must have a Bachelor’s Degree in Human Services, Social Work, Special Education or another related field Must have or be eligible to obtain Ohio DODD SSA certification Must be highly self-motivated, self-disciplined, and able to work cooperatively with others Must have strong communication skills and leadership qualities Must be organized, accurate and have the ability to remain professional and compassionate in stressful situations. Must possess computer skills Must be willing to undergo criminal background checks, work reference checks and drug screening. Two years of experience with children with developmental disabilities preferred Job Summary: Successful applicants will Work one-on-one with children with developmental disabilities and their families, both virtually and in-person Participate in the intake and eligibility process for children Coordinate services and supports using a person-centered approach Responsible for the development and revision of Individual Service Plans (ISPs) for children on caseload Communicate the availability of resources both within the agency and in the community Work cooperatively with schools, community agencies, and team members to meet the needs of children and their families Monitor service provision for quality, continuity, and effectiveness Complete required documentation, maintain confidentiality Maintain up-to-date and complete records of individuals served Attend and lead meetings Complete documentation of all services provided following county guidelines, using designated computer software Hours: Full-time 40 hour per week schedule requiring some flexibility, some ability to work remotely Salary & Benefits: Pay: Commensurate with experience and relevant education.
