JOB SUMMARY **MEMBERS ONLY**SIGN UP NOW*** Home Care Services is seeking a regular part time Home Health Social Worker for our Tazewell Home Health office located onsite at Carilion Tazewell Community Hospital in Tazewell, VA. This position is working 24 hours a week, 8am-4:30pm. This position will support Carilion's hallmarks of service excellence and quality for all job responsibilities. Is an integral part of the care team, contributing professional expertise to support the holistic health of the patient and support for family and/or other caregivers. In collaboration with the health care team the medical social worker assists patients and families with personal and environmental difficulties which predispose illness or interfere with achieving goals of care. Provide patient and families with the psychosocial support needed to cope with disease and life-limiting illness. Services include advising family and caregivers, providing education and support, and making referrals for other services as needed. MINIMUM QUALIFICATIONS
EDUCATION: Bachelor's degree with major studies in social work, sociology or psychology from a four-year college or university accredited by the Council on Social Work Education or 8 additional years of experience required. EXPERIENCE: Minimum of two years social work experience in case work or counseling in a health care or social services delivery system. Experience working with terminally ill patients or home health is preferred. Should possess training and experience in assessing and meeting the emotional and psychological needs of terminally ill clients. Should be able to demonstrate computer proficiency with office suite software. LICENSURE, CERTIFICATION, AND/OR REGISTRATION:Must have valid Virginia Driver's license, use of an automobile with appropriate automobile insurance. Must pass a Criminal Record Check as per Virginia state law. LIFE SUPPORT:Must obtain & maintain AHA BLS-HCP. OTHER MINIMUM QUALIFICATIONS:Effective interpersonal, communication, and team working skills required. Must successfully complete orientation and competency validation for position. Demonstrates advanced knowledge of social work theory and practice: proven expertise as a clinical social worker; crisis intervention skills; critical thinking and creative problem solving. Satisfactory completion of orientation; positive interpersonal skills; oral and written communication skills; analytical skills; cultural competence; integrity; team player; courteous; ability/willingness to work as an integral member of a multi skilled team.
