JOB SUMMARY **MEMBERS ONLY**SIGN UP NOW*** Home Care Services is seeking a full time Home Health RN Case Manager position for the Tazewell Home Health office located in Tazewell, VA. This position is full time working Monday-Friday, 8am-4:30pm with occasional after hours and on call duties.
***SIGN ON BONUS ELIGIBLE*** Will serve as a member of the Home Care team, assumes primary accountability for managing nursing care and provision of individualized care for patients of all ages. Utilizes delegation and critical thinking skills to provide quality service through a team oriented, collaborative approach to patient care. Coordinates care of patient/family unit in collaboration with members of hospice IDT team, Medical Director, and attending physician. Must have exceptional organizational, time management, and documentation skills. May be required to travel in adverse weather conditions. Will be providing patient care in a variety of home settings. MINIMUM QUALIFICATIONS EDUCATION: Graduate of school of professional nursing. EXPERIENCE: For Home Health, one year of experience is required with home health experience preferred. LICENSURE, CERTIFICATION, AND/OR REGISTRATION: Currently licensed as Registered Nurse in the Commonwealth of Virginia or new graduate of approved program eligible to sit for the Virginia licensure exam. For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law. Must obtain /maintain AHA BLS-HCP. OTHER MINIMUM QUALIFICATIONS: Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
