JOB SUMMARY Carilion Home Care Services is seeking a full time Home Health RN Case Manager for the Bedford office located at 35 Medical Court in Hardy, VA. This is a full time position working Monday-Friday, 8am-4:30pm with rotating holidays and weekend and on call requirements. Will work independently seeing patients in a home setting. This position will support Carilion's hallmarks of services excellence and quality for all job responsibilities. As part of the health care team, assumes primary accountability for managing nursing care and provision of individualized care for patients of all ages. MINIMUM QUALIFICATIONS EDUCATION: Graduate of school of professional nursing. EXPERIENCE: For Home Health, one year of experience is required with home health experience preferred. Good computer skills. Able to work independently in a variety of home settings. LICENSURE, CERTIFICATION, AND/OR REGISTRATION: Currently licensed as Registered Nurse in the Commonwealth of Virginia or new graduate of approved program eligible to sit for the Virginia licensure exam. For Home Health, must have valid Virginia Drivers License and pass a Criminal Record Check as per Virginia state law. Must obtain /maintain AHA BLS-HCP. OTHER MINIMUM QUALIFICATIONS: Must successfully complete orientation and competency requirements. Keyboarding skills required for automated computer systems, including electronic medical records. Must be able to meet essential job functions.
