As a member of the Endocrinology and Diabetes Team, participates in clinical, educational, leadership, consultant and research activities related to simple to complex, Type I and II diabetic patients. Provides leadership in the development and implementation of policies, procedures, protocols and standards of practice. Supports the integration and application of evidence-based practice and critical thinking skills to provide optimal healthcare outcomes for critically injured and complex patients. Systems focused, enterprise program facilitator, leader, advisor and mentor.
*Provides leadership in the application of the nursing process based on evidence-based practice, current research and national standards regarding DM management, education and staff/provider training. *Provides highly complex assessment, care planning, care, treatment and discharge/transition planning and placement to specified patients. *Provides leadership in promoting interdisciplinary collaboration to implement outcome-focused patient care needs; creates and manages a DM dashboard that aligns with organizational KPI's and mission. *Develops evidence-based clinical interventions and systems to achieve defined patient and system outcomes. *Coordinates care of patients with use of system and community resources to assure successful health/illness/wellness transitions, enhance delivery of care and achieve optimal patient outcomes.
Interacts frequently with nursing staff, physicians, advanced practice providers, therapists, students and other members of the health care team to coordinate care. Maintains patient, staff and customer confidentiality. Maintains accessibility throughout system and in the community as a resource person. Moderate risk of exposure to infectious diseases, hazardous materials and combative patients. High risk, stressful environment due to demands on time from multiple sources and need to shift priorities frequently. Physical requirements include prolonged standing, reaching, walking, computer use, and limited lifting, bending and pulling.
MINIMUM QUALIFICATIONS EDUCATION: . Master's Degree in Nursing required.
EXPERIENCE: Minimum 5 years of nursing experience. Minimum experience of 1-2 years working with an endocrinologist. Experience with a Diabetologist is preferred.
LICENSURE/CERTIFICATION: Current nursing licensure and specialty certification. Registration as a Clinical Nurse Specialist in the state of Virginia or will obtain within six months of hire. Certified Diabetes Care and Education Specialist (CDCES) required.
