JOB SUMMARY This position is flex time working primarily Monday-Friday with weekend/holiday coverage as needed. Work involves select days to cover for staff on PTO or increased workload. This role supports hospital-based coordinators, facility coordinator, referral review processes within Home Care Services
This position will support Carilion's hallmarks of service excellence and quality for all job responsibilities. The coordinator uses advanced nursing processes in decision-making, patient assessment, data analysis, development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. Provides direct and indirect patient care as needed; oversees the delivery of patient care; and provides day to day management of programs to coordinate patient care as designated by the department. Coordinates the delivery of safe, high quality patient care; identifies areas for study; and participates in and/or supports research. Works in compliance with all State, Federal, and other regulatory requirements. Serves as a resource and mentor to staff and team members. Your accomplishments and expertise are noteworthy. MINIMUM QUALIFICATIONS Education: Graduate of an accredited School of Nursing required. Bachelor's degree preferred. Experience: Minimum of 3 years RN experience required. At least 2 years experience in relevant specialty or department, as defined by the department, desired (ex: hospital setting, vascular, rehab, long term care, home health, cardiology, ambulatory, surgery). Home Health experience required. Licensure, certification, and/or registration: Current VA RN license required. Must successfully complete orientation and competency requirement.
Must obtain and maintain appropriate certifications as defined by the department.
BLS-HCP required within 1 year of hire or at time of hire
ACLS, Conscious Sedation, CCRN, ONC, Pain Management and other modality specific certifications (ex: Cardiology, Surgery, Orthopedics, Vascular) certifications desired. Valid Virginia, or state specific, drivers license required for employees working in Home Health or departments where the work requires traveling.
Other Minimum Qualifications: Effective interpersonal, communication, and team-working skills required. Strong critical thinking, conflict resolution, organization and effective judgement abilities required. Must successfully complete orientation and competency validation requirements. Must be able to meet essential job functions. Designated areas may require Knowledge of Medicare/Medicaid and other third party payor regulations. Must have reliable transportation as travel between facilities required (when permitted post COVID-19). Must be customer-focused; have excellent rapport, have excellent computer and organizational skills and demonstrate flexibility. * Position is deemed emergency personnel during emergency weather conditions.
